Call or email the property to inquire about restrictions on the type of therapeutic animal permitted. The process to have a service or therapeutic animal approved requires: 1. Disability verification form must be signed by applicant/resident and returned to Dublin Office. Management will fax the form to the listed health professionral/clinic to be filled out. 2. If the verification form is returned by the medical professional indicating the need for a service/therapeutic animal the annual vet health certificate form and lease addendums will be completed prior to adding the animal to the lease. The applicant/resident must provide Management with the correct veterinary clinic contact to send the pet health certificate form to.
